If you are concerned about someone who may be experiencing homelessness without being a victim of domestic violence, you can use this form to notify local outreach teams.
How does it work?
Submissions are reviewed and shared with outreach providers serving our region. Outreach workers will attempt to make contact, assess needs, and connect individuals to available services and housing resources.
Before submitting, please note:
This form is not for emergencies.
If someone is in immediate danger, call 911.
This form does not guarantee immediate response or removal of individuals.
Outreach is voluntary – individuals have the right to accept or decline services.
Due to limited capacity, response times may vary.
Please avoid repeated submissions for the same individual unless circumstances change.
